You have got to have some sort of automated testing. I find it totally unacceptable that a bug is fixed, but unknowingly another part of the application has been broken.
Your Developers must have some sort of mechanism where they can fix a bug, change existing functionality or add new functionality and be 100% confident that some other part of the system has not been broken.
